Effect of hypoxia on angiogenesis-related proteins in human dental pulp cells

Abstract

Dental pulp is a highly vascularized tissue with high regenerative potential. Revascularization of severed vasculature in the tooth is required for pulp healing during avulsed tooth treatment. In this study, the relative expression of angiogenesis-related proteins was determined in human dental pulp cells using a human angiogenesis proteome profiler array. The proteome profiler array detected differentially expressed angiogenesis-related factors under conditions of hypoxia, which enhances the angiogenic potential of dental pulp cells. We confirmed that hypoxia regulates the mRNA expression of angiogenesis-related factors, including CXCL16 in dental pulp cells. Furthermore, conditioned media of hypoxic pulp cells induced tube-like structures of vascular endothelial cells, which were reduced by the neutralization of CXCL16 function. In conclusion, our data show that angiogenesisrelated factors are differentially expressed by hypoxia in dental pulp cells and suggest that CXCL16 may involve in the revascularization of hypoxic dental pulp.

    서 론

    치수(dental pulp)는 혈관 분포가 많은 조직이며, 견고 하고 팽창할 수 없는 벽에 둘러싸여 있다[1,2]. 최근에는 치수가 뛰어난 재생력을 가지는 것으로 보고되고 있다 [3]. 치수 절단술 이후, 치수의 치유 과정은 수복상아질 의 생성뿐만 아니라 혈관신생을 필요로 한다[4]. 혈관신 생은 이미 존재하는 혈관에서 새로운 모세혈관이 생성되 는 현상으로써 생리적으로는 배아발생, 상처치유, 조직재 생에 중요한 역할을 한다[5]. 혈관신생은 vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF), thrombospondin-1 등과 같은 다양한 혈관신생 촉 진인자들 및 혈관신생 억제인자들에 의해 활발히 조절된 다고 알려져 있다[5]. 다양한 혈관신생 인자들 중 대표적 혈관신생 촉진인자로 알려진 VEGF 는 건강한 상태와 염 증 상태의 치수세포에서 발현되며 분비되고 있다고 알려 져 있다[6]. 또한 재조합 VEGF 와 bFGF 단백질이 치수 의 미세혈관 밀도를 높여서 상처 난 치수의 혈관재생을 촉진 시킬 수 있다고 보고되었다[7].

    저산소 상태(hypoxia)는 혈관신생을 유도하는 대표적 인 생체 내 신호로, VEGF를 포함한 여러 혈관신생 인 자들의 발현을 유도한다고 알려져 있다[8]. 단단한 조직 에 둘러싸인 치수는 상처에 민감하며, 이러한 상처로 인 한 내압의 증가는 저산소 상태를 일으키기도 한다[9]. 뿐만 아니라 수복과정에서 사용되는 치과용 국소마취제 는 혈관 수축을 유도하여 치수 혈류를 감소시켜, 치수에 서 국소적인 저산소 상태가 관찰된다고도 알려져 있다[4]. 상처로부터 치수를 보호하기 위해 치수는 저산소 상태에서 생리적 반응을 보이는 동시에 병리적으로도 반응하게 된다. 저산소 상태가 치수세포의 세포주기 진 행 차단 및 세포사멸을 유도하기도 하고, 반면에 치수줄 기세포의 증식과 분화를 증가시키며 치수세포의 광화 작용(mineralization)을 촉진시키기도 한다[10,11,12]. 최근에 는 저산소 상태가 치수세포의 혈관신생 잠재력을 촉진 시킨다는 사실이 보고되고 있다[13].

    본 연구에서는 angiogenesis proteome profiler array를 이용하여 치수세포에서 발현되는 혈관신생 인자들을 검 색하고 저산소 상태에서 발현이 달라지는 혈관신생 인 자들을 동정하였다. 발현이 달라지는 혈관신생 인자들 중 CXCL16의 발현이 저산소 상태에 의해 조절되는지 확인하였으며, 저산소 상태가 유도하는 혈관신생 효능에 CXCL16 단백질이 관여하고 있는 지를 검증하였다.

    실험재료 및 방법

    실험재료

    사람의 치수세포(human dental pulp cells)는 일본 Hiroshima 대학의 Takashi Takata 박사 연구실에서 분양 받 아 사용하였다. 치수세포는 10% FBS (Fetal Bovine Serum) 과 penicillin G (10 units/ml), streptomycin (10 mg/ml)이 첨 가된 DMEM 배지로 37℃, 5% CO2 조건에서 배양하였으 며, 3-5일마다 계대 배양하며 유지하였다[14]. 사람의 혈 관내피세포(human umbilical vein endothelial cells, HUVECs) 는 CLONECTICS (Lonza, USA)에서 구매하여 5-8세대 사 이의 세포를 실험에 사용하였다. 0.2% gelatin이 도말된 세포 배양 접시에 10% FBS가 함유된 EGM-2 (CLONTECTICS) 성장 배지를 사용하여 배양하였다.

    Proteome Profiler Human Angiogenesis Array Kit와 CXCL16 중화항체(neutralizing antibody)는 R&D systems 로부터 구입하였다. L-Mimosine과 dimethyloxaloylglycine (DMOG)는 Sigma Aldrich (USA) 제품을 사용하였다. Growth factor-reduced Matrigel은 BD Biosciences (USA) 에서 구 매하였다.

    저산소 상태 (In vitro hypoxia)

    저산소 상태를 만들기 위해 치수세포는 다중가스배양 장치(Anaerobic System Model 1024, Forma Scientific)에서 1% 산소농도로, 표기된 시간만큼 배양하였다. 다중가스 배양장치는 37℃, 95% N2/ 5% CO2 조건을 유지하였다. 이러한 상태는 치수세포의 생존에는 영향을 주지 않음 을 확인하였다. 이러한 조건은 일반적으로 저산소 상태가 유도하는 혈관신생 및 재생연구에 널리 사용되는 조 건이며, 최근 이와 같은 조건에서 치수세포의 혈관신생 잠재력이 증가된다는 사실이 보고된 바 있다[13].

    Proteome Profiler Human Angiogenesis Array

    치수세포는 차가운 PBS로 3회 세척 후, 세포용해액 (40 mM Tris–Cl, 10 mM EDTA, 120 mM NaCl, 0.1% NP-40 및 protease inhibitor cocktail (Sigma Aldrich, USA))으로 용해 시켰다. 각 단백질의 양은 BCA (Sigma Aldrich, USA) 정량 법을 통해 확인하였다. 각 단백질액 (800 μg)은 detection 항체와 잘 섞어준 후, capture 항체가 붙어 있는 membrane 과 반응시켰다. 세척 후, Streptavidin-HRP과 실온에서 반 응시키고, 화학발광제 (ECL: Amersham PharmaciaBiotech) 를 반응시킨 후 X-선 필름 현상으로 단백질 발현을 확인 하였다. 각 반점의 발광 정도는 Image Gauge V4.0 (Fuji film)를 이용하여 수치화하였다. 현상한 X-선 필름을 스캔 하여 각 반점들의 픽셀을 측정하였다.

    RNA 분리와 중합효소연쇄반응 (PCR)

    치수세포에서 Total RNA를 추출하기 위해 TRIzol reagent kit (Invitrogen)를 사용하였다. Reverse transcription kit (Promega, USA)를 사용하여 2μg의 RNA로부터 cDNA를 합성하였다. 합성된 cDNA는 β-actin (sense: 5′-GACTACC TCATGAAGATC-3′, antisense: 5′-GATCCACATCTGCTGGAA -3′), Angiogenin (sense: 5′-TGGGCGTTTTGTTGTTGGTCTTC -3′, antisense: 5′-CGTTTCTGAACCCCGCTGTGG-3′), CXCL16 (sense: 5′-CTGACTCAGCCAGGCAATGG-3′, antisense: 5′ -TGAGTGGACTGCAAGGTGGA-3′), Pentraxin3 (sense: 5′ -GCTGGAGAACTCGCAGATGA-3′, antisense: 5′-CCCAAAT GCAGGCACTAAAA-3′), Coagulation factor III (sense: 5′ -GACAATTTTGGAGTGGGAACCC-3′, antisense: 5′-CACTTTT GTTCCCACCTG-3′)의 primer를 이용하여 역전사중합효소 연쇄반응 (RT-PCR)을 시행하였다. Real-time PCR은 power SYBR Green (Applied Biosystems, USA) 시약을 사용하였 으며, 7500 Real-Time PCR System (Applied Biosystems, USA) 기기를 사용하여 실시하였다. 각각의 primer는 다음 과 같다. β-actin (sense: 5′-ACTCTTCCAGCCTTCCTTCC-3′, antisense: 5′-TGTTGGCGTACAGGT CTTTG-3′), Angiogenin (sense: 5′-GGGCGTTTTGTTGTTGGTCT-3′, antisense: 5′-GC GCTTGTTGCCATGAATAA-3′), CXCL16 (sense: 5′-GGCAGC GTCACTGGAAGTTGTTAT-3′, antisense: 5′-ACCGATGGTAA GCTCTCAGGTGTT-3′), Pentraxin3 (sense: 5′-GTGCTCTCTGG TCT GCAGTG-3′, antisense: 5′-GTCGTCCGTGGCTTGCAG- 3′), Coagulation factor III (sense: 5′-TCCCGAACAG TTAACC GGAA-3′, antisense: 5′-GACCACAAATACCACAGCTCCA-3′).

    관형성 실험 (Tube Formation Assay)

    Growth factor-reduced Matrigel (300 μL)을 24-well plate 에 넣어서 1시간 동안 37℃에서 gel 상태로 굳혀준다. 1.5X105개의 혈관내피세포를 250 μL의 신선한 혈관내피 세포 배지에 준비한다. 여기에 250 μL의 치수세포 배양 액을 섞어준다. 치수세포 배양액은 정상 상태와 저산소 상태에서 8시간 동안 배양 후 얻은 세포배양액을 3000 rpm에서 5분 동안 원심분리하여 불필요한 찌꺼기를 제 거한 상층액을 사용하였다. 전체 500 μL의 정맥내피 세 포가 들어있는 배지를 굳은 gel 표면에 얹어준다. 이후 6시간 동안 37℃ 배양기에서 관이 형성되도록 반응시킨 다. 세포의 형태적인 변화를 현미경으로 관찰하면서 사 진을 찍어둔다. 실험군과 대조군의 관형성 정도 차이를 세어 수치화 한다.

    RNA interference

    치수세포에 OligofectamineTM (Life Technologies)을 이 용하여 HIF-1α의 발현을 선택적으로 저해 할 수 있는 siRNA (antisense: 5´-UCAAACACACUGUGUCCAG-3´; sense: 5´-CUGGACACAGUGUGUUUGA-3´) (100 nM)를 형질주 입 (transient transfection)하였다. HIF-1α siRNA와 control siRNA는 각각 Samchuli Pharm (Korea)와 Bioneer (Korea) 에서 구입하였다. 24시간 형질주입 후, 치수세포는 정상 상태와 저산소 상태에서 표시된 시간만큼 배양하였다.

    결 과

    치수세포에서 혈관신생 관련 단백질의 발현 분석

    사람의 치수세포에서 발현되는 혈관신생과 관련되어 있는 “secretome” 단백질들을 조사하기 위해, 55개의 혈 관신생촉진인자 및 억제인자들의 단백질 발현을 분석할 수 있는 Proteome profiler human angiogenesis array를 실 시하였다. X-선 필름을 이용하여 각 단백질의 발현을 확인하였고, 이를 이미지 프로그램을 이용하여 각 반점 들의 크기를 수치화하였다(그림 1A와 1B). 55개의 단백 질 중 Coagulation factor III, Endoglin, Fibroblast growth factors (FGFs), Insulin-like growth factor-binding protein (IGFBP)-3, Serpin E1, Tissue inhibitor of metalloroteinase (TIMP)-1 및 Thrombospondin-1 단백질들이 높은 수준으 로 발현되어 있음을 확인하였다.

    정상 상태와 저산소 상태에서 치수세포 내 혈관신생 관련 단백질의 발현 패턴

    치수세포는 상아질이라는 경조직에 둘러싸여 있으며, 생리적 또는 병리적으로 산소가 부족한 환경에 노출 되 어있는 경우가 많다. 따라서 저산소 상태에서 혈관신생 관련 단백질들의 발현을 조사하였다. 치수세포를 정상 상태(21% O2)와 저산소 상태(1% O2)로 16시간 동안 배양 하여 치수세포에서 단백질을 각각 분리하였다. 각 단백 질을 Proteome profiler human angiogenesis array kit를 이 용하여 혈관신생 관련 단백질들의 발현 차이를 확인하였 다. 정상 상태와 비교하여 저산소 상태일 경우, 55개의 단백질 중 14개의 단백질이 다른 발현 양상을 보였다(그 림 2A와 2B). 그 중 저산소 상태에서 증가 양상을 보이 는 Angiogenin, Coagulation factor Ⅲ, CXCL16, 및 Pentraxin3 단백질의 발현 차이를 유전자 수준에서 직접 확인하기 위하여 각각의 primer를 이용하여 RT-PCR을 수행하였다(그림 3A). 그림 3A에서 보는 바와 같이 각 유전자 모두 8시간 또는 16시간 동안 저산소 상태에 노 출 된 경우 정상 산소 상태일 때와 비교하여 발현이 증 가됨을 확인 할 수 있었다. 뿐만 아니라 각각의 유전자 들의 양적인 발현 차이를 비교하고자 Real-time PCR을 수행하였다. 그림 3B와 같이 actin의 발현양에 대해 상대적인 발현차이를 비교하였을 때 Angiogenin, CXCL16 및 Pentraxin3의 경우 8시간에서, Coagulation factor Ⅲ는 16 시간에서부터 눈에 띄는 발현차이를 보였다. CXCL16의 경우 8시간 저산소 상태에서 증가하던 유전자 발현이 16 시간에 이르자 감소하는 현상을 나타냈다. 4개의 유전자 모두 RT-PCR과 Real-time PCR을 통해 저산소 상태에서 유전자의 발현이 증가함을 확인하였다. 또한 저산소 상 태와 유사한 환경을 조성하는 L-Mimosine 또는 DMOG를 처리하여 Angiogenin, Coagulation factor Ⅲ, CXCL16 및 Pentraxin3의 발현차이를 확인하고자 하였다. Hypoxiainducible factor-1α (HIF-1α)는 저산소 상태에서 여러 가지 유전자들의 발현을 활성화시키는 대표적인 전사인자이 다[15]. L-Mimosine과 DMOG은 prolyl hydroxylase domain protein의 작용을 방해함으로써 HIF-1α 단백질의 분해를 억제하여 HIF-1α 단백질 발현을 증가시키는 물질들이다 [16]. 그림 3C에서 보이는 바와 같이 정상 상태에 비해 저산소 환경 뿐만 아니라 L-Mimosine과 DMOG를 처리한 경우, Angiogenin, Coagulation factor Ⅲ, CXCL16 및 Pentraxin3의 발현이 증가되어 있음을 확인하였다.

    저산소 상태에서 증가되는 혈관신생 관련 단백질이 혈관내피세포의 관형성에 미치는 영향

    저산소 상태에서 혈관신생 관련 단백질이 증가함을 확 인하였고, 실제 이러한 단백질이 혈관신생에 미치는 영향 을 확인하고자 사람의 혈관내피세포(HUVECs)을 이용하 여 관형성 실험을 실시하였다. 먼저 저산소 상태에서 증 가되는 대표적인 혈관신생 조절 전사인자인 HIF-1α가 미 치는 영향을 확인하고자 치수세포에 HIF-1α siRNA를 형 질 주입하여 HIF-1α의 발현을 저해시켰다. 그림 4A에서 보는 바와 같이 정상 상태의 치수세포의 배양액과 저산소 상태의 치수세포 배양액을 이용하여 혈관내피세포의 관 형성능을 비교하였을 때 저산소 상태의 배양액을 처리한 실험군에서 관형성이 증가 함을 확인하였다. 그러나 HIF-1α의 발현을 억제한 치수세포를 저산소 상태에서 배 양한 실험군에서는 혈관내피세포 관형성능이 감소하는 효과가 나타났다. CXCR6의 리간드인 CXCL16은 다양한 세포들에서 막단백질 형태 및 분비성 단백질로 존재한다. 이러한 CXCL16이 앞서 저산소 상태의 치수세포에서 발 현이 증가되어 있음을 확인하였다. 치수세포 배양액에 CXCL16 단백질의 기능을 억제 할 수 있는 중화 항체 (neutralizing antibody)를 함께 처리한 경우, 저산소 상태의 치수세포 배양액에 의해 증가되었던 혈관내피세포의 관 형성 효과가 억제됨을 확인하였다(그림 4B). 이는 저산소 상태에서 증가된 HIF-1α와 CXCL16이 치수세포 배양액이 유도하는 혈관신생에 중요한 역할을 함을 보여준다.

    고 찰

    치수는 단단한 상아질 벽 내 위치한 느슨한 결합조직 으로 세포, 섬유질, 혈관, 신경을 함유하고 있다. 치수조 직은 치아보호, 영양공급, 수복을 포함한 치아 생명력을 위한 여러 기능을 수행한다[17]. 치수에서 혈관신생은 상처 또는 염증 이후에 치수의 재생과정에 중요한 과정 으로 작용한다[18]. 본 연구를 통해 치수세포는 다양한 혈관신생 촉진인자들 및 억제인자들을 발현하고 있으며, 저산소 상태에서 정상 상태와 다르게 발현되는 혈관신 생 인자들을 관찰하였다.

    건강한 치수 및 염증성 치수조직에서 유래된 치수섬 유아세포가 VEGF를 발현한다고 보고되었다[6]. VEGF 는 생리적, 병리적 혈관신생의 중요한 조절자이며, 혈관 신생을 위한 혈관내피세포의 증식, 이동, 생존 및 관형 성을 유도한다[19]. 특히 최근에는 재조합 VEGF가 면역 이 결핍된 생쥐에 식립된 치아 슬라이스에서 혈관신생 을 유도한다는 사실이 보고되었다[7,20]. 또한 접착 레진, lipoteichoic acid과 같은 여러 가지 외부요인에 의해 서도 치수에서 VEGF의 발현이 유도된다고 알려져 있다 [21]. 본 연구에서 동정된, 치수에서 발현되고 있는 혈관 신생 인자들이 치수에서 생리적, 병리적 다양한 현상에 어떠한 역할을 하는지, 그리고 어떠한 세포 내 신호에 의해 발현이 되고 있는지는 추가 연구가 필요할 것으로 생각된다.

    치수는 높은 재생능을 가지는 것으로 보고되고 있다. 치수는 혈관이 풍부한 조직으로 와동 형성시 치수조직 에 상처가 나면 그 내부의 상처 입은 혈관내피세포는 여러 분자들을 분비시켜 염증반응 및 치유과정을 동시 에 진행시키게 된다[4]. 치수절단술 이후에도 상처에 반 응하여 치수전구세포는 활성화되고 혈관내피세포가 이 동되는 것으로 알려져 있다[22]. 많은 보고들에서는 수 복상아질 형성과정에서 법랑질과 상아질에서 분비되는 성장인자들 및 신호기전 연구에 대해서 집중되어 왔다 [23]. 그러나 치수절단술 이후에 완전한 치수 치유를 위 해서는 수복상아질의 생성뿐만 아니라 혈관신생 및 신 경섬유의 재생도 필요로 한다는 사실이 부각되면서 치 수에서의 혈관신생에 대한 연구가 최근 활발히 이루어 지고 있다[6].

    저산소 상태는 여러 가지 세포에 세포이동, 대사조절, 세포사멸, 혈관신생과 같은 다양한 생리적 반응을 초래 하게 한다[24]. 또한 최근에는 저산소 상태가 세포분화, 전구세포의 증식능, 생존능을 높인다고 보고되고 있다 [25]. 치수세포에서는 저산소 상태가 전구세포의 수를 증폭시키고 혈관신생능 및 상아모세포의 분화능을 높여 효율적인 치료를 유도한다고 in vitro 연구를 통해 알려 져 있다[11,12,13]. 하지만 in vivo에서 저산소 상태가 치수 세포에 직접적인 손상 없이 여러 가지 생리적 현상들을 유발시킬 수 있는지에 대해서 향후 다양한 연구를 통해 증명해야 할 것으로 판단된다.

    본 연구에서는 저산소 상태가 치수세포에서 CXCL16 를 포함한 여러 혈관신생 인자들의 발현을 높이고 있음 을 증명하였다. CXCL16은 케모카인 수용체 CXCR6의 리간드로써, 분비성 단백질 형태뿐만 아니라 막단백질 형태로도 존재한다. CXCL16은 대식세포, 수지상세포, 단핵세포 등에서 발현되며, 다양한 암세포에서 과다발현 된다고 보고되어 있다[26,27]. 많은 연구에서 CXCR6- CXCL16축은 암화과정, 동맥경화 및 염증반응에 주요한 역할을 하는 것으로 잘 알려져 있다[28,29]. 또한 혈관내 피세포의 증식, 관형성, 화학주성을 유발시켜 혈관신생 을 유도하는 혈관신생 촉진인자로 보고되었다[30]. 따라 서 치수의 생리적, 병리적 현상 및 치수 혈관신생에서 CXCR6-CXCL16의 생물학적 역할 및 그 작용기전에 관한 연구는 앞으로 더 진행해야 할 과제로 생각된다.

(A) Control (siCon) and or HIF-1α siRNA (siHIF-1α) was transfected in dental pulp cells for 24 hr. After transfection, these cells were incubated in hypoxia chamber for 8 hr. Conditioned media (CM) were collected from normaxia or hypoxia-conditioned human dental pulp cells and siRNA transfected dental pulp cells. HUVECs were seeded on growth-factor reduced Matrigel and incubated with conditioned media for 4 hr. The tube area number was counted. *P < 0.05 vs. control; #P < 0.1 vs. Hypoxia CM of siCon. (B) HUVECs were seeded on growthfactor reduced Matrigel and incubated with conditioned media (CM) in the presence or absence of neutralizing CXCL16 antibody (20 μg/mL) (α-CXCL16). Conditioned media were collected from dental pulp cells under normoxia or hypoxia. Cells were photographed under a phase contrast microscope and the number of tubes was counted. *P < 0.05 vs. control; #P < 0.02 vs. Hyp CM alone.
