Pre- and Postsynaptic Actions of Reactive Oxygen Species and Nitrogen Species in Spinal Substantia Gelatinosa Neurons

Abstract


Reactive oxygen species (ROS) and nitrogen species (RNS) are involved in cellular signaling processes as a cause of oxidative stress. According to recent studies, ROS and RNS are important signaling molecules involved in pain transmission through spinal mechanisms. In this study, a patch clamp recording was used in spinal slices of rats to investigate the action mechanisms of O2⦁- and NO on the excitability of substantia gelatinosa (SG) neuron. The application of xanthine and xanthine oxidase (X/XO) compound, a ROS donor, induced inward currents and increased the frequency of spontaneous excitatory postsynaptic currents (sEPSC) in slice preparation. The application of S-nitroso-N-acetyl-DLpenicillamine (SNAP), a RNS donor, also induced inward currents and increased the frequency of sEPSC. In a single cell preparation, X/XO and SNAP had no effect on the inward currents, revealing the involvement of presynaptic action. X/XO and SNAP induced a membrane depolarization in current clamp conditions which was significantly decreased by the addition of thapsigargin to an external calcium free solution for blocking synaptic transmission. Furthermore, X/XO and SNAP increased the frequency of action potentials evoked by depolarizing current pulses, suggesting the involvement of postsynaptic action. According to these results, it was estblished that elevated ROS and RNS in the spinal cord can sensitize the dorsal horn neurons via pre- and postsynaptic mechanisms. Therefore, ROS and RNS play similar roles in the regulation of the membrane excitability of SG neurons.

    서 론

    척수후각 표층의 척수 아교질 신경세포(substantia gelatinosa, SG; lamina Ⅱ)는 말초로부터 일차 감각신경섬 유를 통해 직접 정보를 전달받고 이를 통합한다. 신체에 유해한 통각정보는 얇은 수초를 가진 Aδ 섬유와 무수 섬 유인 C 섬유와 같은 감각신경섬유를 통해 척수의 아교질 로 전달되며 이곳에 이웃하는 투사신경세포를 통하여 시 상으로 통각정보를 전달한다[1].

    활성산소종(reactive oxygen species; ROS)은 과산화수소 (H2O2), superoxide 음이온(O2⋅-), 수산기(hydroxyl radical; ⋅OH), 산화질소(NO), 과산화질산염(peroxynitrite; ONOO-) 을 포함하는데, 이러한 활성산소는 조직손상이나 유해반 응과 긴밀히 관련되며[2,3], 또한 유전자 발현, 세포 분화 와 증식 등 세포조절물질로서의 역할이 추가적으로 알려 져 있다[4,5].

    ROS가 통증발생과 유지에 관여된다는 최근의 연구가 보고되었는데, H2O2의 피하투여로 인한 통증유발 연구를 비롯하여 항산화제의 투여로 인한 진통효과가 보고되었 다[6]. 또한 피부 내 capsaicin 투여로 통증을 유도시킨 생 쥐에서 척수후각 뉴론의 미토콘드리아 O2⋅-가 증가되는 결과는[7] 척수후각 뉴론에서 발생된 O2⋅-가 통증을 발생 시키는데 중요한 발생 원인이라고 생각할 수 있다.

    활성질소종 (reactive oxygen species; RNS)의 한 종류인 NO가 통증에 미치는 역할은 통증 유발효과와 진통효과 가 상반되게 같이 보고되어 있다. 예를 들면, 신경손상이 나 조직염증에 의해 유발된 통각과민성은 NOS 억제제를 척수 내로 투여하거나 nNOS 유전자를 삭제한 생쥐에서 감소한 것으로 보고되었으며[8,9], 척수세포에서 발생된 NOS가 만성통증 발생에 중요하게 작용한다고 보고되었 다[10]. 이와는 반대로 또 다른 연구에서는 NO donor인 sodium nitroprusside (SNP)를 발바닥에 주입하면 오히려 진통효과가 유발되었다고 보고되었다[11].

    최근 ROS의 통증조절에 대한 역할을 규명하기 위하여 본 실험실에서는 여러 가지 ROS donor가 척수 아교질 세포의 흥분성에 어떠한 효과를 미치는지에 관한 연구를 진행하였다. Hypochlorite를 발생시키는 NaOCl을 투여하 면 아교질 세포의 막전압의 변화와 세포내 칼슘농도의 증가를 보고하였고[12], NO donor인 SNP인 경우에는 고 농도와 저농도에 따른 막전압에 대한 효과가 서로 다른 것을 규명하였으며[13], 최근에는 ⋅OH를 발생시키는 tertbuthylhydroperoxide를 척수에 주입하여 통각과민을 유도 한 실험동물에서 아교질 세포의 흥분성 시냅스후 전압의 증가를 확인하였다[14].

    이 연구에서는 ROS인 O2⋅-를 발생시키기 위하여 xanthine (X)과 xanthine oxidase (XO)를 복합투여 하였고, RNS인 NO 를 발생시키기 위하여 S-nitroso-N-acetyl-DL-penicillamine (SNAP)을 투여하였다. 발생된 O2⋅-와 NO가 통증전달에 1차 적 중계역할을 하는 척수후각 세포에서 어떠한 기전을 통하 여 흥분성에 영향을 미치는지를 확인하고, 그 효과와 기전 에 차이가 있는지를 patch clamp 방법으로 조사하였다.

    재료 및 방법

    척수절편 제작

    생후 12-17일 된 Sprague-Dawley 흰쥐를 사용하였으며, 실험동물 처치에 관한 사항은 원광대학교 동물실험 윤리 위원회의 규정을 준수하였다. 흰쥐를 ether로 먼저 마취한 후 25% urethane (4 ml/Kg)을 복강 내 투여하였다. 요추부 위를 척추제거술(laminectomy)로 척수를 노출하고 요천수 팽대부에서 1 cm 정도 길이로 척수를 절단하였다. 조직절 편기(vibratome 752M, Campden, GB)를 이용하여 95% O2-5% CO2를 공급하는 상태에서 두께 300 ㎛의 척수절편 을 절단하였다. 척수절편은 32 ℃의 인공 뇌척수액에 30 분 이상 보관하여 정상상태로 회복시켰고, 이후에 실험을 진행하였다. 척수절편을 현미경(BX50WI, Olympus, Japan) 위의 기록용기에 옮긴 후 기록을 시작하였고, 실험과정 동안 지속적으로 95% O2-5% CO2를 공급한 용액을 관류 펌프(Minipuls 3, Gilson, France)를 이용하여 순환시켰다.

    단일 세포 분리

    단일세포에서 전류의 기록은 척수후각 세포에서의 기 록이 성공률이 낮아 척수후각과 같은 기능을 하는 것으로 알려진 연수후각 부위를 이용하였다. 먼저 조직절편기를 이용하여 400 ㎛ 두께로 횡단면 절단하여 뇌절편을 만들 었다. 뇌절편은 단백분해 효소인 protease (Sigma, 0.2 ㎎/㎖) 로 35℃에서 30-50분 동안 처리하고, thermolysin (Sigma, 0.2 ㎎/㎖)에 같은 온도로 15분간 처리하였다. 효소처리가 끝난 후 연수후각 부위는 21G 주사침을 이용하여 분리하 였고, 이를 지름 1 cm의 유리판 위에서 작은 피펫속으로 흡입, 분출하는 과정을 반복하여 단일세포로 분리하였다.

    실험용액

    실험에 필요한 세포외 용액의 조성은 117 NaCl, 3.6 KCl, 2.5 CaCl2, 1.2 MgCl2, 1.2 NaH2PO4, 25 NaHCO3, 11 glucose 이었고 95% O2-5% CO2를 공급하여 pH를 7.4 상 태로 유지하였다. 세포내 용액은 150 K-Glu, 10 HEPES, 5 KCl, 0.1 EGTA, 5 MgATP, 0.3 Na GTP를 사용하였고, pH 는 KOH를 이용하여 7.3으로 조정하였다.

    실험에 사용된 xanthine(X), xanthine oxidase (XO), S-nitroso- N-acetyl –DL-penicillamine (SNAP), thapsigargin, glutamate, capsaicin은 Sigma (USA)에서 구입하였다. Thapsigargin은 DMSO (dimethyl sulfoxide; Sigma)에 일차적으로 녹인 후 최종농도로 세포외 용액과 혼합하여 사용하였고, 그 밖의 다른 약물은 실험 전에 세포외 용액에 녹여 사용하였다. 실험용액의 관류는 중력을 이용한 관류장치(BPS-4SG, Ala Scientific Instruments, USA)를 이용하였다.

    전기생리학적 기록방법

    막전압과 전류의 기록을 위하여 whole cell patch clamp의 전류고정법과 전압고정법을 각각 사용하였다. 미세 유리전극 제조기(PP-830, Narishige, Japan)를 이용하 여 외경 1.5 ㎜의 유리관(TW150-3, WPI, USA)을 저항이 5-8 ㏁이 되도록 미세전극을 제작하였다. 저배율 렌즈로 관찰하였을 때 밝은 띠를 형성하는 척수 아교질 부위를 확인한 후 양압을 가하면서 미세 전극조절기(ROE-200, Sutter, 미국)를 이용하여 세포에 접근하였다. Seal test를 시행하면서 세포에 접근하여 피펫의 저항이 순간적으로 급증하는 것으로 세포에 근접하였음을 확인하고 음압을 가하여 세포와의 gigaohm seal을 이루었다. 다시 짧은 음압을 가하여 whole cell을 만들었으며 막전압이 -45 mV 이하로 안정된 상태에서 기록을 시작하였다. 전압과 전류측정은 Axopatch 200B 증폭기(Axon, USA)를 사용하 였으며, 이 증폭기는 Digidata 1200B (Axon, USA) AD변 환기를 통해 컴퓨터와 연결하였다. pCLAMP software (version 9.0, Axon, USA)를 사용하여 데이터의 저장 및 분석을 시행하였으며 발생된 전류는 low pass 8-pole Bessel filter로 2 ㎑로 여과하였다. 모든 실험은 실온에 서 시행하였다.

    실험자료의 분석

    막전압과 전류의 분석은 Clampfit (version 9.0, Axon, USA)를 사용하였으며, 흥분성 시냅스후 전류의 분석은 Mini Analysis program (Synaptosoft, 미국)을 이용하였다. 시냅스 전류 빈도 확인은 10-15 pA 이상의 크기를 가진 전류의 97% 이상이 포함되게 하였다. 약물처리군 사이에 통계적으로 유의한 차이가 있는지의 여부는 independent t-test를 이용하였고 약물처리 전후비교는 paired t-test를 이용하였으며, p<0.05에서 통계적으로 유의하다고 판정하 였다. 통계자료의 값은 평균값±표준오차 (mean±SEM)로 표시하였다.

    연구성적

    X/XO, SNAP 투여에 의한 시냅스 전달에 대한 작용

    X/XO와 SNAP 투여에 의해 발생된 ROS가 시냅스 전 뉴론의 신경전달물질 분비에 영향을 주는지 혹은 시냅스 후 뉴론에 직접 작용하는지를 알아보았다. 이를 위하여 자발적 흥분성 시냅스후 전류(sEPSCs)에 대한 효과, 단일 세포 상태에서의 반응, 시냅스 전달을 차단하기 위한 칼 슘통로 억제제의 처리 등의 상황에서 약물효과를 확인하 였다.

    sEPSCs에 대한 X/XO와 SNAP의 효과를 관찰하기 위하 여 유지전압을 -70 mV로 고정하고 지속적으로 시냅스 전 류를 기록하였다. 발생된 시냅스 전류는 하향파형이 기록 되었는데, X/XO (300 μM/30 mU)를 투여하였을 때 약물 투여 전에 비하여 sEPSCs의 빈도가 증가됨이 관찰되었고 (0.48±0.05 Hz vs. 0.71±0.09 Hz, n=10, p<0.01), 크기에서는 유의한 변화가 없었다(13.0±1.1 pA vs. 14.5±1.3 pA, n=10) (Fig. 1).

    SNAP (500 μM)를 투여하였을 때에도 약물투여 전에 비하여 sEPSCs의 빈도가 증가됨이 관찰되었고(0.52±0.06 Hz vs. 0.69±0.07 Hz, n=9, p<0.05), 크기에서는 유의한 변 화가 없었다(12.1±0.8 pA vs. 13.2±1.3 pA, n=9) (Fig. 2).

    시냅스의 영향을 완전히 배제하기 위한 방법으로 단백 분해 효소 처리에 의해 단일 세포로 분리한 후 X/XO와 SNAP를 투여하였다. 효소처리에 의해 분리된 연수후각 세포의 크기는 15 μm에서 25 μm까지 다양 하였으며, 형 태는 구형과 타원형이 많이 관찰 되었다. X/XO의 투여에 의해 기록한 7개의 세포에서 모두 내향성 전류를 기록하 지 못하였고, SNAP에 의해서는 기록한 8개의 세포에서 2 개의 세포에서만 5 pA 정도의 내향성 전류가 기록되었다. 단일세포로 분리한 세포가 약물에 잘 반응하는지 확인하 기 위하여 흥분성 신경전달 물질인 glutamate를 투여하였 을 때 기록한 모든 세포에서 내향성 전류가 확인되었고 (18.2±2.5 pA, n=5), capsaicin 처리에 의해서는 4개의 세포 에서 모두 전류가 기록되지 않았다(Fig. 3).

    전류고정법으로 막전압의 변화를 기록하면서 시냅스 차단의 효과를 관찰하였다. 정상 세포외용액에서 X/XO 투여에 의한 탈분극을 기록하고 세포외 용액에서 Ca2+를 제거하고 세포막 칼슘통로를 억제할 수 있는 Cd2+를 첨가 시킨 용액에서 X/XO에 투여에 의한 탈분극과 비교하였 을 때 유의한 변화를 보이지 않았다 (7.3±0.5 mV, n=14 vs. 5.2±1.0 mV, n=6) (Fig. 4A, D). 세포내 Ca2+은 세포내 칼슘저장소인 내형질세망(endoplasmic reticulum; ER)으로 부터 증가될 수 있으므로 ER의 Ca2+ ATPase 억제제인 thapsigargin을 세포외 Ca2+ 제거용액에 첨가하여 저장소내 칼슘을 고갈시킨 후 X/XO를 투여 하였을 때는 X/XO에 의한 탈분극이 유의하게 억제됨이 관찰되었다(1.3±0.8 mV, n=7, p<0.001) (Fig. 4B, D). 세포외 Ca2+ 제거용액에 thapsigargin을 첨가한 용액에서 SNAP를 투여 하였을 때 에도 SNAP 단독 투여에 의한 탈분극에 비해 유의하게 억 제됨이 관찰되었다(6.3±0.6 mV, n=14 vs. 2.7±1.2 mV, n=7, p<0.05) (Fig. 4C, D).

    시냅스후 뉴론의 흥분성에 대한 작용

    X/XO와 SNAP가 시냅스 전달에 작용하는 것 외에 시 냅스후 뉴론에 직접 영향을 미치는지 확인하기 위하여 아 교질 세포에 양전류를 가하여 활동전압을 발생시킨 후 X/XO와 SNAP를 투여하였다.

    척수 아교질 신경세포는 지속적인 양전류를 가하여 탈 분극 시켰을 때 발생하는 활동전압의 순응정도에 따라 두 가지로 구분한다[15,16]. 아교질 세포에 25-50 pA의 양전 류를 기록전극을 이용하여 단계적으로 자극하였을 때 활 동전압이 기록되었는데, 이를 1-3 초의 자극기간 동안 활 동전압의 빈도가 감소하는 adapting firing neuron (AFN)과 자극기간 동안 지속적이고 반복적인 활동전압을 보이는 tonic firing neuron (TFN)으로 구분하였다. Adapting firing neuron은 발생되는 활동전압의 빈도가 높지 않아 이 연구 에서는 tonic firing neuron만 실험에 사용하였다. X/XO처 리에 의해서 25, 50 pA 자극에서 각각 6.8±1.5 Hz vs. 9.2±1.1 Hz (n=9, p<0.05), 16.4±2.1 Hz vs. 18.2±1.8 Hz (n=9)로 증가하였고, SNAP 처리에 의해서도 25, 50 pA 자 극에서 각각 7.3±1.6 Hz vs. 10.7±1.8 Hz (n=8, p<0.05), 18.2±2.2 Hz vs. 21.4±1.5 Hz (n=8, p<0.05)로 증가하였다 (Fig. 5).

    고 찰

    ROS는 조직손상이나 유해반응에 의하여 세포내에서 증가하며 세포독성을 일으킴으로서 세포사 유도에 영향 을 미친다. 또한, 유전자 발현, 세포 분화와 증식 등 여러 가지 생리적인 기능을 조절할 수 있는 세포조절물질로서 의 역할도 보고되었다[5,17]. 최근에는 ROS가 통증발생에 직접적으로 관여되어 있다고 보고되었는데 예를 들면 척 수의 ROS 양이 감소함에 따라 통증행동 반응이 감소되었 고[18], ROS 양이 증가함에 따라 그에 따른 통증 반응이 비례적으로 증가됨이[19,20] 보고되었다. 특히 척수 미토 콘드리아의 superoxide dismutase (SOD) 양에 영향을 받으 며[20], tirilazad[21], PBN[6], vitamin E[22] 같은 다양한 항 산화제를 전신투여 혹은 척수내로 투여함으로서 진통효 과가 나타나는 것이 보고되었다. 이러한 결과들은 척수에 서 작용하는 활성산소가 통증의 발생과 유지 및 전달에 중요하게 관여하고 있음을 시사한다.

    이 연구에서는 ROS를 발생시키기 위해 X와 XO를 혼 합하여 투여하였다. XO는 X을 산화시켜 uric acid로 만드 는 과정에서 O2⋅-을 발생시키는 것으로 알려져 있고 이것 은 세포 내 SOD에 의해 H2O2로 변환된다. 이 연구결과에 서 X/XO에 의한 ROS는 척수후각 뉴런에서 내향성 전류 와 탈분극을 유발하고 활동전압의 빈도수 증가를 발생시 켰다. Sato 등[23]과 Hawkins 등[24]은 X/XO 투여에 의해 약물투여 직후부터 O2⋅-가 발생되기 시작하여 3분에서 최 고치를 보이고 5분정도 경과하면 SOD에 의해 H2O2로 전 환된다고 하였으며, Zhou 등[25]도 X/XO 투여는 즉각적 이고 일시적인 변화를 보이지만 H2O2 투여는 느리고 지 속적인 변화를 보인다고 하였다. 이 연구에서도 X/XO는 약물 투여 후 1분 이내부터 즉각적인 내향성 전류와 탈분 극이 관찰되었으며 이는 이전에 보고한 H2O2 투여에 의 해 발생되는 작고 느린 반응과는 차이가 있었다[26].

    이 연구에서는 약물적용 시간을 5분으로 하였는데 이 는 O2⋅-가 SOD에 의해 H2O2로 전환되기 충분한 시간으로 X/XO의 투여는 O2⋅- 외에 H2O2도 발생시켜 세포에 영향 을 미쳤을 것으로 사료된다. 다른 연구에서도 H2O2는 세 포막을 쉽게 확산으로 통과하여 막전압을 변화시킬 수 있 다고 보고하였는데[19,27], transient receptor potential (TRP) 통로를 활성화하거나[28], ATP 민감성 K+(KATP) 통로의 열림을 조절하여[29] 막전압을 변화시킨다고 하였다.

    이 연구에서 전압고정법으로 시냅스전류를 기록하면서 X/XO와 SNAP를 투여 하였을 때 내향성 전류와 흥분성 시냅스후 전류의 빈도 증가가 발생되었다. 척수 아교질 세포에서 ROS가 시냅스 신호전달을 조절하는 것에 관한 연구는 Takahashi 등[30]이 H2O2가 GABA 작동성 미세 억 제성 시냅스후 전류(miniature inhibitory postsynaptic potential; mIPSC)를 증가시킨다고 하였고, 반대로 Yowtak 등[31]은 ⋅OH donor인 t-BOOH가 IPSC를 감소시킨다고 보고하였을 뿐 흥분성 시냅스후 전류에 대한 결과는 보고 되고 있지 않다. 그러나 해마의 신경세포와 대뇌피질의 신경세포에서 과산화수소 투여에 의해 세포외 glutamate 농도가 증가됨이[32] 확인되었고, 흰쥐 척수에서 과도한 glutamate의 축적은 지속적으로 glutamate 수용체의 과활 성을 초래하여 자발적인 통증반응을 보인다고 하였다 [33]. 이 연구에서 X/XO에 의한 O2⋅-와 SNAP에 의한 NO 가 신경말단에서 glutamate를 증가시키고 내향성 전류를 유발할 수 있음은 흥분성 시냅스후 전류의 빈도가 증가한 결과와 단일세포에서 내향성 전류가 발생하지 않은 결과 등을 통하여 확인하였다.

    이 연구에서는 X/XO와 SNAP에 의해 TFN에서 양전류 자극에 의해 발생하는 활동전압의 빈도가 증가됨을 관찰 하였다 (Fig. 5). 대부분의 TFN은 광범위 동작성 신경세포 (wide dynamic neuron)와 유해 특이성 신경세포(nociceptive specific neuron)로 통증에 반응하는 세포로 구성된다[15]. 그러므로 이 연구의 결과는 X/XO와 SNAP가 주로 통증을 전달하는 세포에 작용하여 활동전압의 역치와 근접해서 활성화되는 전압의존성 통로의 활성화에 영향을 주었을 가능성이 있다. TFN에서는 전압의존성 Na+통로와 지연성 정류형 K+ (KDR) 통로가 활동전압을 발생시키는데 관여하 고 있으며 Ca2+활성화 K+전류 (KCA)도 부가적으로 빈도를 조절한다[34]. 다른 세포에서도 과산화수소는 KDR[35]과 KCA[36] 등과 같은 전압의존성 통로에 영향을 주어 신경 흥분성을 조절함이 보고되었다.

    이상의 연구결과를 종합하면 xanthine과 xanthine 산화 제 투여에 의해 발생된 활성산소와 SNAP 투여에 의해 발 생한 산화질소는 척수후각의 아교질 세포를 탈분극 시킴 에 의해 흥분성에 영향을 미칠 수 있으며, 이는 시냅스 전에 작용하여 신경전달물질의 양을 증가시키거나 시냅 스후 뉴론의 이온통로에 직접 작용하여 탈분극 시킬 수 있음을 확인하였다.

    Blockade of synaptic transmission decreased X/XOand SNAP-induced membrane depolarization.

    (A) X/XO-induced membrane depolarizations were not significantly reduced by a combination of CdCl2 and calcium free solution. When membrane potential was recorded in an extracellular calcium free solution with thapsigargin, X/XO -induced depolarization was markedly decreased (B), and SNAP-induced depolarization was significantly decreased (C). (D) Mean X/XO- and SNAP-induced depolarization in control and in neurons pretreated with Ca2+ free plus thapsigargin. ***: Values are significantly different from the control by independent t-test (p<0.001).     Effects of X/XO and SNAP on neuronal excitability of the SG neurons.

    (A) Recordings of action potentials generated in a tonic-firing neuron (TFN). Action potentials were evoked by direct intracellular injections of current pulses (25 pA steps). After X/XO treatment, the frequency of action potentials was increased by current pulse of 25 pA and 50 pA. (B) After SNAP treatment, the frequency of action potentials was increased by current pulse of 25 pA and 50 pA. Average firing rate measured by step current pulses (25, 50 pA) after X/XO treatment (C) and SNAP treatment (D). *: Values are significantly different from the control by paired t-test (p<0.05).
