Expression of vesicular glutamate transporter in transient receptor potential vanilloid 1-positive neurons in the rat trigeminal ganglion

Abstract


Activation of transient receptor potential vanilloid 1 (TRPV1), a calcium permeable channel expressed in primary sensory neurons, induces the release of glutamate from their central and peripheral afferents during normal acute and pathological pain. However, little information is available regarding the glutamate release mechanism associated with TRPV1 activation in primary sensory neurons. To address this issue, we investigated the expression of vesicular glutamate transporter (VGLUT) in TRPV1-immunopositive (+) neurons in the rat trigeminal ganglion (TG) under normal and complete Freund’s adjuvant (CFA)-induced inflammatory pain conditions using behavioral testing as well as double immunofluorescence staining with antisera against TRPV1 and VGLUT1 or VGLUT2. TRPV1 was primarily expressed in small and medium-sized TG neurons. TRPV1+ neurons constituted approximately 27% of all TG neurons. Among all TRPV1+ neurons, the proportion of TRPV1+ neurons coexpressing VGLUT1 (VGLUT1+/ TRPV1+ neurons) and VGLUT2 (VGLUT2+/TRPV1+ neurons) was 0.4% ± 0.2% and 22.4% ± 2.8%, respectively. The proportion of TRPV1+ and VGLUT2+ neurons was higher in the CFA group than in the control group (TRPV1+ neurons: 31.5% ± 2.5% vs. 26.5% ± 1.2%, VGLUT2+ neurons: 31.8% ± 1.1% vs. 24.6% ± 1.5%, p < 0.05), whereas the proportion of VGLUT1+, VGLUT1+/TRPV1+, and VGLUT2+/TRPV1+ neurons did not differ significantly between the CFA and control groups. These findings together suggest that VGLUT2, a major isoform of VGLUTs, is involved in TRPV1 activation-associated glutamate release during normal acute and inflammatory pain.

    Introduction

    Transient receptor potential vanilloid 1 (TRPV1)은 칼슘이온투과 성 양이온채널로서, 유해열자극(> 43℃), 캡사이신(capsaicin), 양성 자(proton) 그리고 알릴이소티오시아네이트(allyl isothiocyanate) 등 에 의해 활성화되며, 급성 통증 및 염증성 통각과민 유발에 중요한 역할 을 한다[1,2]. TRPV1은 삼차신경절(trigeminal ganglion)과 뒤뿌리신 경절(dorsal root ganglion)에서 작거나 중간 크기의 신경세포에서 발 현되며[3-5], 통각수용성 일차 감각신경세포의 표지자로 알려져 있다 [6]. 정상 흰쥐의 spinal cord slices에 capsaicin을 가하면 glutamate 성 sEPSC (spontaneous excitatory postsynaptic current)의 빈 도가 증가하며[7], 말초염증을 야기한 흰쥐의 spinal cord slices에 TRPV1 억제제를 가하면 glutamate성 sEPSC의 빈도가 감소한다[8]. 이는 정상 및 염증상태에서 TRPV1이 활성화되면 중추성 신경섬유에서 glutamate 분비가 항진되며, 이는 TRPV1 매개 통증에 중요한 역할을 할 것임을 시사한다. 그러나 TRPV1 활성화 시 어떤 기전에 의해 일차 감각신경세포로부터 glutamate가 분비되는지에 대해서는 잘 알려져 있지 않다.

    Vesicular glutamate transporters (VGLUTs)는 glutamate를 연 접소포에 탑재하여 분비하는데 관여하며, glutamate성 신경세포의 표 지자로 사용되고 있다[9,10]. VGLUT의 3가지 isoform 중 VGLUT1과 VGLUT2는 대부분의 일차감각 신경세포에서 발현되며 이들은 각기 다 른 종류의 신경세포에서 발현된다. 즉, VGLUT1, VGLUT2를 발현하는 일차 감각신경세포의 크기, 중추 내 투사 양식, substance P, calcitonin gene-related peptide (CGRP) 등과 같은 유해수용기의 발현 등 으로 미루어 보아 VGLUT1은 주로 저역치기계자극 수용기를 발현하는 신경세포에서, VGLUT2는 유해수용기를 발현하는 신경세포에서 발현 하는 것으로 생각되고 있다[9,11-15]. 또한, VGLUT2–/– 마우스를 사 용하여 이의 기능을 분석하는 실험에서 VGLUT2는 급성 통증, 염증성 통증, 신경병성 통증의 전도에 관여하는 것으로 알려져 있다[16-18].

    이 연구에서는 정상 상태에서 TRPV1에서 발현하는 VGLUT의 유형 을 분석하고, 염증성 통증이 유발된 상태에서 TRPV1과 VGLUT의 발 현 변화를 분석하여 TRPV1의 활성화와 연관된 glutamate 분비 기전 을 이해하고자 하였다. 이를 위해 다중면역형광법을 시행하여 정상 및 염증성 통증 상태 흰쥐의 삼차신경절 신경세포에서 TRPV1, VGLUT1 과 VGLUT2의 발현을 분석하였다.

    Materials and Methods

    1. 실험동물

    실험동물 보호와 처치는 경북대학교 동물실험윤리위원회(Intramural Animal Care and Use Committee)의 승인에 따른 동물실험계획서 에 의거하여 진행하였다(Approval number: KNU-2017-139). 이 연 구를 위해 9마리의 흰쥐(Sprague-Dawley rat, 300–320 g)를 사용 하였다: 3마리의 흰쥐는 정상 삼차신경절을 분석하는 데 사용하였으며, 3마리의 흰쥐는 대조군으로 생리식염수 주입에 사용하였고, 나머지 3 마리는 complete Freund’s adjuvant (CFA, heat-killed mycobacterium tuberculosis; Sigama-Aldrich, St. Louis, MO, USA) 주입 에 의한 염증성 통증유발 실험군으로 사용하였다. 실험동물의 행동분석 에서 열자극에 의한 통증반응이 큰 4일군에서 CFA 주입 실험군 3마리, 생리식염수 주입 대조군 3마리를 대상으로 면역형광조직화학법 수행 및 분석을 수행하였다.

    2. 염증성 통증 동물모델에 대한 행동분석

    실험동물을 ketamine (40 mg/kg)과 rompun (10 mg/kg) 혼합용 액으로 근육 마취한 후 대조군과 실험군에 각각 40 μL의 생리식염수와 CFA 용액(phosphate buffered saline [PBS], 0.1 M PBS로 1:1 희 석, 20 mg/mL)을 좌측 vibrissa 부위의 피하 부위에 주입하였다: 이 전 연구에서 흰쥐 vibrissa 영역에 CFA 주입 1일째부터 열과민성 통증 이 발생하고, 5일째에 가장 유의하였으며, 14일째부터 CFA를 주입하 기 이전 수준으로 회복된다는 것이 보고된 바 있다[19]. 본 연구는 이전 실험을 참고하여 행동분석 실험을 진행하였다. 레이저 자극기(infrared diode laser, LVI-810-30; LVI Technology, Seoul, Korea)를 이용 하여 좌측 vibrissa 영역에 열자극을 가한 다음, 열자극으로부터 얼굴을 피하는 시간(head withdrawal latency)을 측정하여 평가하였다. 자극 은 5분 간격을 두고 3번을 가한 다음 평균값을 산출하였으며, 조직손상 을 방지하기 위한 cut-off time은 15초로 설정하였다.

    3. 면역형광조직화학법을 위한 조직 준비

    행동분석에서 실험동물이 열자극에 의한 통증반응이 강한 4일군에 서 CFA 주입 실험군 3마리, saline 주입 대조군 3마리를 대상으로 면 역형광염색법을 위한 조직 준비를 하였다. 실험동물을 sodium pentobarbital (60 mg/kg, intraperitoneal)로 깊이 전신마취하고, 오름 대동맥을 통하여 100 mL의 헤파린이 함유된 생리식염수로 혈액 제거 후, 400 mL의 4% paraformaldehyde (PFA, in 0.1 M phosphate buffer: PB, pH 7.4) 고정액으로 관류고정을 시행하였다. 이후 좌측 삼 차신경절을 분리한 뒤 동일 고정액에 2시간 후고정하였다. 동결 시 조 직손상을 최소화하기 위해 30% sucrose 용액에 약 16시간 동안 처리 한 후, 동결절편기를 사용하여 삼차신경절을 30 μm 두께로 절편 제작 후 면역형광염색법을 시행하였다.

    4. 면역형광염색법

    면역형광염색법의 모든 반응은 실온에서 시행하였다. 항체의 침투를 높이기 위해 50% ethanol에 조직절편을 침적하였으며, 10% normal donkey serum (NDS, Jackson ImmunoResearch, West Groove, PA, USA)에서 반응하여 비특이적 반응을 최소화하였다. 그 후 TRPV1 항체와 VGLUT1 혹은 VGLUT2 항체의 혼합용액에서 16시간 동안 실 온에서 반응하였다. 본 연구에 사용된 일차항체들은 다음과 같다: goat anti-TRPV1 antibody (1:400, SC-12498; Santa Cruz Biotechnology, Dallas, TX, USA), rabbit anti-VGLUT1 antibody (1:1,000, #135303; Synaptic Systems, Gottingen, Germany), guinea pig anti-VGLUT2 antibody (1:200, Af810; Frontier Institute Co. Ltd, Hokkaido, Japan). 이후 0.01 M PBS로 세척, 2% NDS로 반응 후 이차항체(cyanine (Cy3)- 혹은 fluorescein isothiocyanate (FITC)- conjugated antibodies)들을 이용하여 3시간 반응시켰다. 그 후, 0.01 M PBS와 증류수로 세척한 다음 형광용 슬라이드 위에 절편을 올린 후 형광현미경(Zeiss Axioplan 2; Carl Zeiss, Gottingen, Germany)으 로 관찰하였다. 현미경 영상은 형광현미경에 장착된 디지털 카메라(QImaging Inc., Surrey, CA, USA)로 촬영하였다. 이들 현미경 영상은 TIFF 확장자 파일로 저장하였으며, Adobe Photoshop 7.0 (Adobe System, San Jose, CA, USA)을 이용하여 명도와 대비를 조정하 였고, ImageJ software (v1.38; National Institutes of Health, Bethesda, MD, USA)를 사용하여 분석하였다.

    5. 면역형광조직화학법에 사용된 항체들의 특이성 조사

    본 연구에서 사용된 일차항체의 특이성을 확인하기 위해 대조실험을 진행하였다. 일차항체 혹은 이차항체를 제외한 다음, 앞에서 언급한 것 과 동일한 방법의 면역형광염색법을 시행한 결과 TRPV1, VGLUT1, VGLUT2 면역양성반응이 관찰되지 않았으며, 이는 본 연구에 사용된 TRPV1, VGLUT1, VGLUT2 항체가 특이성이 있다는 것을 의미한다 (Fig. 1).

    6. 면역형광염색법 결과의 정량적 분석

    삼차신경절에서 신경세포의 핵이 명확하고 세포질에 면역형광반응이 균등하게 보이는 신경세포를 면역양성반응으로 결정하여 분석에 사용 하였다. 면역양성 신경세포의 단면적은 3마리에서 각 삼차신경절당 3 개의 비연속 절편들을 선택하여 분석하였다. 실험동물별로 나타난 측정 결과는 서로 유의한 차이가 없었으며, 신경세포 크기(단면적)에 대한 결 과들은 통합하여 분석하였다. 면역양성 신경세포는 크기에 따라 다음 과 같이 분류하였다: 작은 신경세포(< 500 μm2 in cross-sectional area), 중간 크기의 신경세포(500–1,000 μm2), 큰 신경세포(> 1,000 μm2). 위턱부위를 지배하는 삼차신경절의 영역에서 TRPV1 면역양성 신경세포가 많이 관찰되는 부위를 무작위로 선택하여 총 20장을 200배 율(860.76 × 655.70 μm, 1,360 × 1,036 pixels)로 촬영하여 분석 하였다. 면역양성 신경세포의 밝기는 255 gray levels에서 100–120 정도로 역치를 조정하였다. 삼차신경절의 위턱부위에서 전체 신경세포 중 TRPV1, VGLUT1, VGLUT2 면역양성 신경세포수 및 TRPV1 면 역양성 신경세포 중 VGLUT1 (VGLUT1+/TRPV1+) 또는 VGLUT2 (VGLUT2+/TRPV1+)를 발현하는 신경세포 수를 정상군, 실험군에서 측정한 뒤 mean ± standard deviation (SD)로 표시하였다. 각 신경 세포 비율의 변화는 Student’s t-test를 사용하여 통계학적 분석과 비 교를 실시하였으며, 통계적 유의성의 표준값은 p < 0.05로 정하였다.

    Results

    1. 삼차신경절에서 TRPV1 발현 신경세포의 VGLUTs 공존

    정상 상태의 흰쥐 삼차신경절에서 TRPV1, VGLUT1, VGLUT2 면 역양성 반응은 신경세포의 세포질에서 관찰되었고 핵에서는 관찰되 지 않았다. TRPV1은 작거나 중간 크기의 신경세포에서 주로 발현되 었다(mean ± SD in cross-sectional area: 453.7 ± 170.2, size range: 126.6–1,319.1 μm2, Fig. 2A and 2B). TRPV1 발현 신경세 포의 활성화 시 어떤 유형의 VGLUT를 통하여 흥분성 신호전달물질인 glutamate를 방출하는지를 이해하기 위해, TRPV1과 VGLUT1 또는 VGLUT2 항체들을 이용하여 이중면역형광염색을 시행하여 다음과 같 은 결과를 얻었다: TRPV1 면역양성 신경세포는 삼차신경절 전체 신경 세포의 약 1/4 (26.5 ± 1.2%)을 차지하였고, TRPV1 면역양성 신경 세포 중에서 VGLUT1을 발현하는 신경세포(VGLUT1+/TRPV1+)는 거의 관찰되지 않았으며(0.4 ± 0.2%), VGLUT2를 발현하는 신경세 포는(VGLUT2+/TRPV1+, %) 약 1/4 (22.4 ± 2.8%)을 차지하였다. VGLUT1+/TRPV1+ 신경세포는 중간 크기의 세포였으며, VGLUT2+/ TRPV1+ 신경세포는 주로 작은 크기의 세포였다(Fig. 2C and 2D).

    2. 염증성 통증 동물 모델

    실험동물의 좌측 vibrissa 영역에 CFA를 피하조직으로 주입한 다음 레이저에 의한 열자극 회피반응 시간(head withdrawal time)을 측정 한 결과 CFA 주입 1일째부터 열자극에 대한 회피반응 시간이 생리식 염수를 주입한 대조군에 비해 유의하게 감소되었고, 이는 CFA 주입 후 14일까지 지속되었으며, CFA 주입 후 4일째에 회피반응 시간이 가장 크게 감소하였다(Fig. 3).

    3. 염증성 통증 유발군에서 TRPV1과 VGLUT 발현의 변화

    열자극에 의한 통증과민 반응이 가장 큰 CFA 주입 4일군 실험동물의 삼차신경절에서 TRPV1+, VGLUT1+과 VGLUT2+ 신경세포의 비율을 분석한 결과는 다음과 같다. 삼차신경절의 위턱지배부위에서 전체 신 경세포 중 TRPV1+, VGLUT2+ 신경세포의 비율(fraction, %)은 CFA 군에서 대조군에 비해 유의하게 증가하였다(TRPV1+ 신경세포: 31.5 ± 2.5% vs. 26.5 ± 1.2%, VGLUT2+ 신경세포: 31.8 ± 1.1% vs. 24.6 ± 1.5%, p < 0.05, Fig. 4). 그러나 VGLUT1+, VGLUT1+/ TRPV1+, VGLUT2+/TRPV1+ 신경세포의 비율은 CFA군과 대조군 사 이에 차이가 없었다(VGLUT1+ 신경세포: 22.9 ± 1.3% vs. 24.3 ± 1.2%, VGLUT1+/TRPV1+ 신경세포: 0.3 ± 0.1% vs. 0.4 ± 0.2%, VGLUT2+/TRPV1+ 신경세포: 25.8 ± 0.9% vs. 22.4 ± 2.8%, Fig. 4).

    Discussion

    본 연구의 주된 결과는 다음과 같다. 1) 흰쥐의 삼차신경절에서 TRPV1+ 신경세포의 다수가 VGLUT2를 발현하였으며 VGLUT1은 거 의 발현하지 않았다. 2) 염증성 통증을 보이는 CFA군 삼차신경절에서 대조군에 비해 TRPV1+, VGLUT2+ 신경세포의 비율은 증가하였으나, VGLUT1+, VGLUT1/TRPV1+, VGLUT2/TRPV1+ 신경세포의 비 율은 유의한 차이가 없었다. 이러한 결과는 1) TRPV1에 의해 매개되 는 급성 및 염증성 통증과 관련이 있는 TRPV1의 활성화에 의한 glutamate 분비에는 VGLUT의 유형 중 주로 VGLUT2가 관여할 것이며, 2) 염증 상태에서 TRPV1 활성화 증가에 의한 VGLUT2 활성화 증가, 이로 인한 말초 및 중추에서의 glutamate signaling의 증가가 TRPV1 매개 통증 과민화와 관계가 있을 것이라는 것을 시사한다.

    1. 정상 삼차신경절 TRPV1+ 신경세포의 VGLUT 발현

    TRPV1은 유해열자극(> 43℃), capsaicin과 proton 등에 의해 활성 화되고, 급성 통증, 병적 통증의 유발에 중요한 역할을 한다[1,2]. 일차 감각신경세포 혹은 신경섬유에서 발현되는 TRPV1을 capsaicin 등에 의해 활성화하면 이들의 중추성 신경섬유에서 glutamate가 분비되어 통증전도에 관여한다고 알려져 있다[7,8,20,21].

    본 연구에서 TRPV1+ 신경세포의 다수(22.4%)가 VGLUT2를, 극소 수(0.4%)가 VGLUT1을 발현하였다. 이는 정상상태에서 TRPV1에 의 해 매개되는 두개안면 영역의 열자극, 혹은 capsaicin 투여에 의한 급성 통각정보의 중추 내 전달에는 VGLUT1보다는 VGLUT2에 의해 매개 되는 glutamate 분비가 주로 관여할 것이라는 것을 시사한다. 정상상 태에서 삼차신경절 TRPV1+ 신경세포의 대다수(약 77%)는 VGLUT1, VGLUT2를 발현하지 않았다. TRPV1+ 일차감각 신경세포가 glutamate를 신경전달물질로 사용한다는 것[21]을 감안하면, TRPV1 활성 화에 의해 야기되는 glutamate 분비 및 이와 관련된 통증정보의 전달 에는 주로 VGLUT1, VGLUT2와 연관이 없는 glutamte signaling 기 전이 관련되어 있을 가능성이 있다. 즉, 다수의 TRPV1+ 신경세포는 1) VGLUT3에 매개되는 glutamate 분비에 관여하거나(VGLUT3는 소수 의 일차감각 신경세포에서 발현되므로 존재하더라도 glutamate분비 에 대한 기여가 미약할 것임), 2) Ca에 의존하지 않는 비전통적 방식에 의한 glutamate성 신경전달기전[22]이 관련되어 있을 수 있다. 또한 TRPV1+ 신경세포가 glutamate 이외의 신경전달물질에 의한 신경전 달기전[23,24]에 연관될 가능성도 배제할 수 없다.

    2. 염증성 통증 시 삼차신경절 신경세포에서 TRPV1과 VGLUT2 발현의 증가

    흰쥐 말초조직에 염증을 야기하면 열자극에 대한 통증반응이 증가하 며, 척수뒤뿔과 등뿌리신경절에서 TRPV1 단백질과 mRNA 수준이 증 가한다[25-27]. 또한, 말초염증 시 TRPV1 길항제인 SB-366791을 처리하면 척수뒤뿔에서 glutamate성 연접전달이 억제되며[8], 뒤뿌리 신경절의 신경세포에서 선택적으로 VGLUT2가 소실된 마우스는 염증 시 기계적 자극, 열자극에 의한 반응이 감소한다[16-18]. 이러한 연구 들은 TRPV1과 VGLUT2가 염증성 통증의 전도에 중요한 역할을 할 것 이라는 것을 시사한다.

    본 연구에서 이러한 연구들과 유사하게, CFA 주입에 의한 두개안면 만성염증성 통증유발 시 삼차신경절 신경세포에서 TRPV1, VGLUT2 의 발현이 증가하였다. 특히 VGLUT2+/TRPV1+ 신경세포의 비율 은 변화가 없었는데, 이는 염증성 통증 시 TRPV1+ 신경세포 수의 증 가와 비례하여 이들 세포에서 VGLUT2의 발현이 동일 비율로 증가함 을 의미한다. 이러한 결과는 염증성 통증 시 다양한 염증성 물질에 의해 TRPV1 활성화가 증가하며, 이는 TRPV1+ 세포 내 다양한 단계를 통하 여 VGLUT2의 발현 증가를 야기하고, 이어서 TRPV1+ 신경세포의 중 추성, 말초성 신경섬유에서 glutamate signaling의 증가로 인한 중추 성, 말초성 민감화에 기여함으로써 염증성 통증에 기여할 것으로 생각 한다. 이러한 연구결과는 TRPV1 억제제와 함께 VGLUT2 억제제의 동 시 투여가 TRPV1 매개 염증성 통증의 억제에 효과적일 것이라는 생각 을 하게 한다.

    뒤뿌리신경절의 신경세포는 크기에 따라 큰 세포, 중간 크기 세포 와 작은 세포로 나눌 수 있다. 이들은 각기 큰 말이집신경섬유(large myelinated Aβ fiber), 작은 말이집신경섬유(small myelinated Aδ fiber), 민말이집신경섬유(unmyelinated fiber)를 가지며, 촉각과 같 은 저역치 기계자극, 부위를 정확히 알 수 있는 예리한 통증, 부위를 정 확히 알 수 없는 둔통을 전달하는 데 관여하는 것으로 알려져 있다[28- 30]. 본 연구에서 TRPV1+ 신경세포 중 VGLUT2를 발현하는 신경세포 는 대부분이 작은 크기(< 500 μm2 in cross-sectional area)의 신경 세포였다. 이는 TRPV1의 활성화, VGLUT2 활성화에 의한 glutamate 의 분비기전에 관련된 TRPV1 매개 급성 및 만성통증정보는 대부분 민 말이집 C 신경섬유를 통해 전도될 것이라는 것을 시사한다.

    Time course of head withdrawal latency following injection of complete Freund’s adjuvant (CFA) and vehicle into the rat left vibrissa pad. Thermal pain hyperalgesia was observed at various time points after CFA injection into rat vibrissal pad. CFA-treated rats show significantly reduced head withdrawal latency time to heat stimulation compared to the vehicleinjected rats. Each value is presented as mean ± standard error. Asterisks indicate a statistically significant difference between CFA and vehicle.

Double immunofluorescent staining for TRPV1 and VGLUT1 (A, B) or VGLUT2 (C, D) and proportion (%, E) of VGLUT1+, VGLUT2+, TRPV1+, VGLUT1+/TRPV1+ or VGLUT2+/TRPV1+ neurons in control (A, C) vs. the CFA-treated (B, D) groups. The proportion of TRPV1+, VGLUT2+ neurons is significantly higher in the CFA-treated group compared to control (n = 3 animals per group).
